Routine appointments
If you need a routine nurse or healthcare assistant appointment
Please ring to make an appointment and be prepared for the receptionist to ask you for the reason you need an appointment so we can give you an appropriate time slot. A member of staff is available to act as a chaperone at your appointment if required or deemed necessary by a clinician. All our calls may be recorded for training and monitoring purposes.
Pregnancy Self-Referral
For patients who need to register with Jessop Wing for their pregnancy please use the self referral form which can be found on the Jessop Wing website – https://jessopwingmaternity.sth.nhs.uk/self-referral.php

You can contact us about a routine medical, administrative or prescription issue by clicking here.
Answer a few short questions and we will get back to you within 1 working day. Please do not use this online form for urgent requests. All our calls may be recorded for training and monitoring purposes. The submitted forms will only be read during practice opening hours. The forms will not be read on weekends (Saturdays and Sundays) or on Bank Holidays. Please contact 111 during out of hours or 999 if this is an emergency. A member of staff is available to act as a chaperone at your appointment if required or deemed necessary by a clinician.

Weekend and evening appointments are available for our patients.
Remote and face-to-face appointments can be made with a variety of GP practice staff including Doctors, Advance Care Practitioners, Nurses, Nurse Associates, Health Care Assistants and Physiotherapists.
Appointments can be booked following triage with the practice.

Home Visits
Where possible, please phone Reception before 10am to enable your request to be triaged.
A clinician may phone you back as it may be that your problem can be dealt with by telephone advice. It sometimes is more appropriate to send a nurse, or indeed arrange a hospital attendance. All our calls may be recorded for training and monitoring purposes.
Home visits are only available for patients who are housebound because of illness or disability.
