Insurance/Critical Illness cover

Typically, when you apply for products such as life insurance and critical illness cover your insurance company would normally pay for the doctor’s time to complete this work should it be required.

Other form completion requests

Forms that need completing by patients which require a doctors input or signature are chargeable and must be met by the patient. These requests include:

• Westfield forms
• Holiday cancellation forms
• Private sick notes
• Statement of fact
• Short and long reports
• HGV Medicals

For any non NHS requests, please allow at least 2 weeks for completion.
