Greystones Medical Centre acknowledge there may be occasions when your information or contact details are used in a way that does not involve providing direct, current care to you.

This includes, but is not limited to:

  • Summary Care Records
  • Text messaging system
  • Sharing of your information with other services (such as physiotherapists) who are currently providing direct care to you.

Informed written consent is required before any of these systems are used. The only exceptions to this is the Summary Care Records systems which is a national system which assumes consent unless a patient has dissented.

Any patient is welcome to complete and sign this Your Electronic Patient Record and sharing of information form in order to let us know your wishes.

What is the National Data Opt-Out

The national data opt-out gives everyone the ability to stop health and adult social care organisations from sharing their confidential patient information for reasons other than providing their individual care and treatment. The national data opt-out only applies where the data processing relies upon Regulation 5 of the Control of Patient Information Regulations 2002.

Individuals can view or change their national data opt-out choice at any time by using the online service at www.nhs.uk/your-nhs-data-matters or by calling 0300 303 5678 (Monday to Friday, 9am to 5pm)
