Register with our practice

Anyone in England can register at a GP Surgery.  You do not need proof of address or immigration status, ID or an NHS number.

All new patients wishing to register with the Practice must live within the Practice boundary as set by Dr Benn and approved by NHS Sheffield. You can check whether or not you live within the Practice boundary using the map below.

You can register using our online service called Register with a GP Surgery that makes it easy to register with this GP surgery, just click the link below.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

New patients are advised that any repeat medications cannot be ordered until you have seen a GP or a Pharmacist.

Named GP

As a patient at Greystones Medical Centre, your named and accountable GP will be Dr Richard Benn.

Accessing someone else's information

As a patient, you can give authorisation for another individual to have access to your medical record in certain circumstances.  In order for us to record this correctly on your record, you will need to give your consent for us to do this.  Please use the form below highlighting the level of authorisation you wish to give and return it to the surgery.
